What is Your Plan for Health (YP4H)?
YP4H is Ohio State’s approach to fostering a culture of well-being and optimal performance by providing benefits-eligible faculty, staff, and their families with the programs and resources that will empower them to pursue a life of health and wellness.
Who is eligible for Your Plan for Health (YP4H)?
Benefits-eligible faculty/staff and spouses/same-sex domestic partners, COTC employees, OSUP employees and graduate students enrolled in an Ohio State medical plan are eligible to participate in YP4H.

Where should I look to make sure I’m receiving the Medical Plan Premium Credit?
To see if you’re receiving the PHA Credit in your pay, look at your pay stub under Earnings and Hours. If you’re receiving the credit, PHA Credit will be listed; $20/monthly pay or $10/biweekly pay. If your enrolled spouse/same-sex domestic partner completed the PHA, this PHA Credit will be listed as $10/monthly pay or $5/biweekly pay.
Can I participate in Your Plan for Health (YP4H) if I’m not enrolled in an Ohio State medical plan?
Even though you are not enrolled in an Ohio State medical plan, benefits-eligible faculty/staff can participate in YP4H. For more information on how you can participate, visit yp4h.osu.edu/welcome.
Can dependent children participate in Your Plan for Health (YP4H)?
Dependent children are not eligible to participate in YP4H.
What is an on-campus Biometric Health Screening?
Complimentary Biometric Health Screenings can be scheduled by benefits-eligible faculty/staff and enrolled spouses/same-sex domestic partners to meet with a nurse and obtain health values. These consist of height, weight, blood pressure, total cholesterol, HDL and A1C.
How do I schedule an on-campus Biometric Health Screening?

Where and when are on-campus Biometric Health Screenings offered?
Biometric Health Screenings are offered at convenient locations on Ohio State main and regional campuses on various days and times. To view a list of available locations, days, and times, go to Start Here at the top and click Schedule a Biometric Screening.
How recent do my health values need to be to complete the Personal Health & Well-Being Assessment (PHA)?
Health values received during the current calendar year can be used to complete the PHA.
